No. Your wallet is your account. You sign one free message to prove it is yours, pay in USDC on Arc, and the card is issued to that wallet. There is no identity check and no paperwork.
USDC on Arc. USDC is Arc's native token, so the payment is a single pre-filled transfer from your wallet and gas is paid in USDC as well. Funds on other networks do not count, so bridge to Arc first.
A flat $15 issuance fee plus whatever balance you want on the card, starting at $10. Top-ups are $15 plus the amount, from $10 up to $1,000 at a time. There is no monthly fee.
